Diaphragmatic rupture presented with herniation and strangulation of the abdominal viscera in the thoracic cavity
Traumatic diaphragmatic hernia are often undiagnosed, and can be discovered in case of complications such as obstruction. We reported a case of delayed presentation of post traumatic diaphragmatic rupture with herniation and strangulation of stomach, transverse colon, and the jejunum of a 52-year-old patient who had a fracture of acetabulum six month back following road traffic accident. The diagnosis was suspected based on the clinical features of obstruction and air- filled viscera in the left hemithorax. Diagnosis was confirmed by an abdominal and thoracic computerized tomography (CT) scan.Exploratory laparotomy revealed a diaphragmatic rupture with strangulation of the stomach, transverse colon and the jejunum. The treatment consisted of the reduction of the herniated organs and closure of the diaphragmatic defect.

Inverted Takotsubo (about a case with literature review)
Takotsubo cardiomyopathy is characterized by transient systolic dysfunction of LV apical midventricular segments in the absence of obstructive coronary artery disease. Inverted Takotsubo is a very rare variant in which a patient’s transthoracic echocardiogram that we report revealed the characteristic basal and midventricular segmental kinetic disorders. The non-association with coronary artery disease can be differentiated with the use of echography by evaluating the temporal changes of akinetic areas and coronarography by showing the normality of the coronary arteries

Postpartum subcapsular hepatic hematoma related to preeclampsia: Conservative management
Subcapsular hematoma of the liver represents an unusual clinical phenomenon in the pregnancy and postpartum period. Serious complications can be devastating in terms of fetal and maternal mortality. We present a case of a multiparous patient of 44 -year-old at 37 weeks of gestation, admitted for preeclampsia. The woman underwent an emergency Cesarean section (C-section) with the extraction of a live foetus. Few hours after delivery, she complained of intense abdominal and epigastric pain. Diagnostic work-up was suggestive of a subcapsular right lob hepatic hematoma which was successfully managed conservatively in a multidisciplinary team. We will try through this case to highlight the interest of imaging (ultrasound and scanner) for an early diagnosis at the non-ruptured stage, a prerequisite for a good prognosis.

Ankle Block in Orthopedic Surgery: A Review
Foot surgery is characterized by intense postoperative pain, difficult to control even when using balanced multimodal analgesia associating paracetamol, anti-inflammatory drugs and opioids. Ankle blocks were all but abandoned. Complications are rare using regional anesthesia for postoperative analgesia even after extensive foot surgery, however the consequences of motoric or sensory deficits can be extremely devastating as it impacts mobility.Revival of ankle blocks are a perfect example of the high impact of new technological advances in improving ambulatory surgical care after foot surgery.

Comparison of 6 minute walk test (6MWT ) and 6 minute step test (6MST ) and its effect on % maximal heart rate in football players : An observational study
The Objective of the study was to compare the % maximal heart rate achieved by the subjects at the end of each test to verify whether or not % maximal heart rate achieved by post 6 minute step test is more than 6 minute walk test 6MWT and 6MST were performed according to the American Thoracic Society guidelines. Blood Pressure, Respiratory Rate, Oxygen Saturation, Heart Rate and %Maximal Heart Rate were measured and recorded. The Cross Sectional Observational Study performed on Local Football Club , Pune. The heart rate post 6MWT increased significantly but within normal range (p <0.04).The mean % MHR pre-test was 38% and post-test was 42%. The heart rate post 6MST increased significantly too but within normal range. The mean %MHR pre-test was 37% and post-test was 47%. (p <0.01). As there is a significant increase in the %MHR post 6 minute step test in football players, it can be concluded that the two tests i.e. 6 minute walk test and 6 minute step test are not interchangeable in the following population. This also concludes that the 6 minute step test can be used to increase the cardiovascular endurance in Football Players.
